Click on the “Call for Dental Services” link on the top right corner of the form.3.
The first time you call, you will be asked to confirm that you are a “Cancer Survivor,” and to give your age.4.
A doctor will be called and the dental health will be checked.5.
You can then go ahead and select a date and time for your appointment.6.
You will then be directed to a separate appointment room and you will receive a confirmation email.7.
You should check the box on the first page of your confirmation email that says “Call Now.”8.
After the initial call, the doctor will tell you whether or not you have any questions.9.
The doctor will then contact you via phone or email, and the appointment will begin.
You can call at any time, 24 hours a day, seven days a week, and you can call to schedule your appointment anytime.
If you choose to schedule a new appointment, you may want to call ahead to see how long the appointment is taking to be completed.
The dentist will fill out the appointment in person.
Once your appointment is complete, the dentist will check your health and tell you if you have cancer.
You then receive a checkup.
Once you return home, you can schedule a follow-up appointment.
The dentist will then ask you to send in your current medical records.
If you have more questions, you should call the doctor directly.
The appointment may take up to two weeks.
You should call if you:1.
Are a smoker or are overweight or obese2.
Have had an accident or been hit by a car4.
Have a chronic health condition or illness5.
Have certain conditions, including high blood pressure6.
Have any other health conditions or are in an early stage of cancer7.
